Job Description

Position Summary

Provide personalized care and support to clients in the comfort of their homes, ensuring a high-quality, compassionate service that meets the individual needs of each client. Work closely with the client and their family to promote independence, safety, and overall well-being.

Key Responsibilities

• Personalized Care : Assist clients with daily activities such as bathing, dressing, grooming, and personal hygiene, while encouraging their independence.
• Nutritional Support : Plan and prepare healthy, nutritious meals tailored to the client's preferences and dietary needs. Assist with feeding if necessary.
• Household Support : Light housekeeping duties including laundry, cleaning, and organizing to maintain a safe, comfortable, and welcoming home environment.
• Companionship : Provide meaningful social interaction, engaging conversation, and emotional support to improve the client's overall quality of life.
• Health Monitoring : Observe and report any changes in the client's condition, alerting the supervisor or healthcare professionals as necessary.
• Documentation : Maintain accurate records of services provided, including dates and times, ensuring clear communication with the healthcare team.
• Specialized Care : Under the guidance of a physician or healthcare professional, assist with more specialized tasks as outlined by the Colorado Nurse Aide Practice Act.
• Other Duties : Assist with a range of tasks as needed, ensuring the well-being and comfort of the client.
