Job Summary
Under general supervision, the Medical Aid in Dying Program Coordinator plans, organizes, and implements social work treatment for terminally ill patients accessing the Colorado End of Life Options Act through Denver Health while also supporting their next of kin. The Program Coordinator assesses the biopsychosocial needs of patients and families and creates an individual plan of care, as it relates to the patient's current health concerns and ongoing access to care outside of DH. Provides clinical interventions that help meet identified needs by utilizing personal, family and community resources. Provides crisis intervention, supportive counseling, case consultation and care coordination services. The Program Coordinator is also responsible for executing administrative tasks related to patients completing access to Medical Aid In Dying as required by the law.
Essential Functions :
Facilities end of life conversations with terminally ill patients and their families, including education related to the Colorado End of Life Options Act and the MAiD Clinic at Denver Health. (20%)
Collects necessary medical records related to patient care as required by Colorado law for the Medical Aid in Dying process and registers / schedules all patients in Epic for the MAiD Medical Director. (20%)
Applies knowledge of individual, group and family functioning to ensure appropriateness of treatment methods to assist patient in achieving care plan goals. Demonstrates the ability to develop effective brief interventions that are socio-culturally sensitive. (5%)
Routinely consults, negotiates, and coordinates with internal and external resources to assure collaborative efforts to maximize patient outcomes (5%)

Skilled in utilizing Epic, specifically registering and scheduling patients for appointments.
Ability to request and manage medical records relevant to the Colorado End of Life Options Act.
