Description :

This is a 12-week position.

Case managers work either from 7 : 00 AM to 3 : 30 PM or from 8 : 00 AM to 4 : 30 PM based on unit needs.

The case managers cover all units except for psychiatric and women's and infants.

Tasks include completing assessments on all admitted patients.

Attending interdisciplinary rounds is required.

Initiating referrals for home health, durable medical equipment, skilled nursing facilities, and post-acute care is part of the responsibilities.

Average daily census is over 1000 hospital beds.

Experience in high acuity discharges and discharge planning is necessary.

Candidates should be able to assess needs, coordinate care, and expedite appropriate time frames for testing and transition of care.

Additional flexibility to float within service lines is required.

Candidates must have two years of experience in case management in a facility of similar capacity.

Academic experience is preferred.
