Your Role in Patient Care
As a Home Health Registered Nurse, you will provide compassionate and skilled care to patients in their homes. You will work collaboratively with a multidisciplinary team to enhance patient outcomes and ensure the highest standards of care.
- Conduct thorough assessments of patients’ physical and psychological needs, documenting findings diligently.
- Complete required assessments and visit documentation according to agency policies.
- Share critical observations with the Clinical Manager and physicians to coordinate effective care.
- Supervise and guide home health aides and LPNs/LVNs, ensuring quality service delivery.
- Participate in an on-call rotation as per agency policy.
Qualifications
- Graduation from an accredited nursing program.
- A minimum of one year of experience in home health, public nursing, or acute care nursing.
- Knowledge of Medicare home health regulations and ICD-10 coding is highly preferred.
- Valid driver's license and reliable transportation with necessary insurance.
- Current BCLS Certification is required before patient care provision.
