Position Overview
We are looking for a dynamic and organized Clinical Manager to oversee clinical operations, provide leadership to field clinicians, and enhance workflow efficiency through the Home Care Home Base (HCHB) EMR system. This hybrid role includes both administrative oversight and the flexibility to perform in-home visits and participate in the on-call rotation . The Clinical Manager will also play a central role in building, recruiting, training , and leading a strong clinical team.
Key Responsibilities
Provide clinical oversight, support, and mentorship to RNs, LPNs, therapists, and other field staffFacilitate daily clinical workflow using Home Care Home Base (HCHB) to ensure timely and compliant documentationPerform in-home visits as needed to support patient care and staff coverageRecruit, onboard, and train new clinical team members, fostering a culture of professionalism and excellenceCollaborate with the Coding team to ensure accurate documentation and proper claim submissionLead weekly clinical team meetings , reviewing key agency performance metrics, including :SOC (Start of Care) timelinessROC (Resumption of Care) and Recertification complianceSTAR reports and QAPI (Quality Assurance and Performance Improvement) dataTeaching and training updates and staff development needsMonitor clinical documentation and ensure compliance with agency, state, and federal guidelinesParticipate in on-call rotation and assist with care coordination during non-business hoursPartner with QA, intake, and scheduling teams to optimize patient outcomes and field operationsQualifications
Active RN license in the state of [Insert State]Minimum of 2 years of clinical home health experience; supervisory or managerial experience preferredStrong proficiency with Home Care Home Base (HCHB) EMR systemExperience leading team meetings and analyzing clinical / operational data preferredExcellent communication, leadership, and problem-solving skillsAbility to perform occasional field visits and participate in the on-call rotationValid driver's license and reliable transportationBenefits
