Job Description
Traveling to and working within both Union and Somerset Counties. Please note that travel to these counties is a required part of the position .
1. Performs comprehensive psychosocial assessments
- Identifies behaviors and issues that affect coping at the end of life
- Identifies issues that may have an impact on the safety of patients and families Identifies availability of support systems to meet patient / family goals
- Identifies bereavement risks and communicates these appropriately
2. Collaborates with other disciplines in the development of the plan of care
Provides input to the development of short-term and long-term goalsParticipates as an active member of the interdisciplinary teamCollaborates with external resources in developing the plan of care3. Collaborates with other disciplines in the implementation of the plan of care
Counsel's patients and families in strategies to cope with issues of loss and terminal illnessParticipates with other disciplines in family conferencesProvides referrals to appropriate community servicesFacilitates movement between levels of hospice care4. Collaborates with other disciplines in the implementation of the plan of care

Licensed in the state of NJ(LSW)MSW DegreeExpertise in community resourcesExcellent customer service and communication skillsKnowledgeable regarding Hospice Conditions of ParticipationPreferred -
