Community Connector (Everett / Marysville area)

Molina Healthcare  hiring is for a  Field-Based Community Connector  in or around the Everett or Marysville area.

This role will be the non-clinical support for the Health Care Services team. Duties will include (but not be limited) to the following :

• Be the "Face of Molina" to our members. Meeting with them in their homes, or in the community to ensure they are getting the care and services needed. This includes introducing them to the Molina Case Management team and following up with members who have been hard to connect with.
• This role can expect  50% + / - local travel , meeting with members in the field. (Must have a valid driver's license and a reliable vehicle).
• Reaching out to members (both in person and on the phone) to conduct post discharge screening. Referring members to the appropriate departments depending on the outcome of the screening.
• Assisting with Health Risk screenings.
• Assisting with reviewing additional program and special projects as needed and other duties as assigned.

REQUIRED EDUCATION :  HS Diploma / GED

PREFERRED EDUCATION :  Associate degree in a health care related field (e.g., nutrition, counseling, social work).
