Qualifications

Develop and implement individualized care plans based on current and prior medical historyDeliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understandingConduct thorough assessments of physical, emotional, and spiritual needs throughout the care journeyAccurately monitor and document vital signs including temperature, pulse, respiration, and blood pressureAdminister physician-ordered medications and treatments with skill and empathyEducate patients and their families on care techniques and symptom managementTrack and communicate changes in patient condition, ensuring timely and effective responsesKeep detailed and up-to-date clinical notes to reflect patient progress and quality of care providedCollaborate with interdisciplinary teams to ensure comprehensive care deliveryAnd contribute to other supportive duties as part of a compassionate, mission-driven teamAnd additional duties as assignedWhat You'll Bring to The Team :
