Job Description

Job Title : RN Supervisor

Job Specialty : Long-Term Care and Post-Acute Care

Job Duration : Permanent

Shift : 12-hour Days, 7 a.m. to 7 p.m.

Guaranteed Hours : Full-time

Experience : Experience in dementia care, chronic illnesses, diabetes management, and rehabilitation preferred.

License : New York State RN License

Certifications : BLS Certification

Must-Have : Experience with Electronic Medical Records (EMR), specifically PointClick EMR.

Job Description :

• Oversee the care provided in a 272-bed long-term care and post-acute care facility.
• Supervise various units including two secure dementia care units.
• Manage post-acute rehabilitation patients, primarily from hospital transfers.
• Attend to patients requiring different levels of care, such as continuous BiPAP therapy, no ventilators, and no new tracheostomies.
• Administer tube feedings, manage Foley catheters, IVs, PICCs, and ports.
• Provide care for fresh post-operative patients, including hip and knee surgeries and elective procedures.
• Perform wound care and administer nebulizer treatments.
• Ensure no new admissions are processed past 3 p.m.
• Serve as the sole RN supervisor during off shifts (nights, weekends, holidays), attending to acute changes like falls or emergencies requiring ER visits.
• Conduct head-to-toe assessments and coordinate with healthcare providers to facilitate efficient patient transfer to emergency departments if needed.
