Key Responsibilities :

Deliver in-home therapy services according to PT plans

Participate in interdisciplinary assessments and update treatment plans

Ensure thorough documentation of interventions and patient progress

Educate patients and family members for home therapy adherence

Must Have + Certifications :

Current MN Physical Therapist Assistant license

Preferred 2+ years related PTA experience

Valid driver’s license, insurable transportation

Current CPR / BLS certifications
