SUMMARY :

Join our dedicated team as a Licensed Practical Nurse in a Long-Term Care setting, where you'll play a critical role in providing compassionate and professional care to our residents. In this dynamic position, you will oversee and guide the Certified Nursing Assistants (CNAs) in delivering high-quality care, ensuring that documentation and caregiving protocols are consistently adhered to for the best outcomes.

RESPONSIBILITIES :

Administer medications accurately and in a timely manner, while maintaining proper documentation.

Perform prescribed treatments and document residents' progress following physician orders and facility protocols.

Ensure comprehensive resident care documentation using our computerized medical record system, initiating appropriate care pathways.

Communicate relevant information and reports to appropriate staff as per departmental protocols.

Monitor resident conditions and report changes promptly to the charge nurse, supervisor, or physician.

Conduct pre-reviews of medication administration records according to facility guidelines.

Complete required Minimum Data Set (MDS) documentation and observations in a timely manner.

Assist the CNA team with direct personal care of residents, fostering teamwork and cooperation.

REQUIRED QUALIFICATIONS :

Graduation from an accredited School of Practical Nursing.

Current New York State Licensure as a Practical Nurse.

PREFERRED QUALIFICATIONS :

At least one year of experience in long-term care is preferred.

EDUCATION :

LICENSES / CERTIFICATIONS :

BLS - Basic Life Support from the American Heart Association, LPN - Licensed Practical Nurse from the New York State Education Department.

PHYSICAL REQUIREMENTS :

This position requires medium physical exertion, including the ability to lift 20 to 50 pounds occasionally, and greater than negligible lifting (up to 10 pounds) constantly. Frequent walking, standing, or squatting is also necessary.
