Professional Summary

The Forms Coordinator plays a vital role in supporting clinical operations by managing and processing essential documentation related to patient leave and disability. This includes Family and Medical Leave Act (FMLA) forms, Short and Long-Term Disability paperwork, and Return to Work documentation. The ideal candidate will possess a strong understanding of relevant regulations, exceptional organizational skills, and the ability to communicate effectively with both patients and healthcare providers. This role ensures timely and accurate form completion, contributing to a seamless patient experience and regulatory compliance.

Key Responsibilities

  • Manage and complete FMLA, Short-Term Disability, Long-Term Disability, and Return to Work forms for patients.
  • Interpret clinical chart notes and translate them accurately into required documentation.
  • Communicate professionally with patients and healthcare providers to gather necessary information and ensure form accuracy.
  • Maintain compliance with HIPAA and OSHA regulations in all documentation and communication.
  • Track and prioritize multiple form requests to ensure timely processing and delivery.
  • Collaborate with clinical staff to ensure accurate and complete documentation is available for form completion.
  • Maintain up-to-date knowledge of FMLA and disability guidelines to ensure regulatory compliance.
  • Utilize Epic EMR and Microsoft Office tools to manage documentation and communication efficiently.

Qualifications

  • High school diploma or equivalent required.
  • Medical Assistant Certification or comparable clinical experience preferred.
  • Experience with FMLA, Disability, and Return to Work forms and procedures strongly preferred.
  • ROI (Release of Information) experience is a plus.
  • Proficiency in Microsoft Office (Excel, Word, Outlook).
  • Experience with Epic EMR preferred.
  • Knowledge of medical terminology and ability to educate patients as needed.
  • Strong organizational skills and attention to detail.
  • Effective written and verbal communication skills.
  • Passion for sports performance and experience supporting orthopedic sub-specialties is a plus.

    The pay range for this position is $22.00 - $35.00 / hr.
