• Coverage : ASAP‐ongoing
• 7a‐3p / 4‐5 days per month
• Office visits, follow‐up visits, procedures
• Level 4 Trauma
• 15 PPD
• EMR : Epic
• Board Certified
• Active WI license or IMLC required
