Home Health Care Clinical Manager - Registered Nurse (RN)

• Facilitate ongoing interdisciplinary assessments of patients.\n
• Guide and support home care clinicians to ensure compliance and quality of care according to all federal and state regulations.\n
• Supervise both paraprofessional and licensed professionals in the team.\n
• Utilize clinical decision-making skills to effectively manage a designated patient population.\n
• Develop educational programs aimed at empowering patients, families, and caregivers to achieve healthcare goals.\n\nQualifications : \n
• Active Registered Nurse (RN) license in Colorado (BSN preferred).\n
• Minimum of three (3) years of nursing experience in home healthcare; management experience is a plus.\n
• Previous experience with OASIS preferred.\n
• Current CPR Certification (course demonstration required).\n
• Strong clinical proficiency and excellent communication skills.\n
• Ability to work collaboratively in a team-oriented environment and take initiative.\n
• Familiarity with State and Federal home care regulations.\n
