As a Medication History Pharmacy Technician, you will play a crucial role in ensuring high-quality patient care by accurately compiling medication histories for patients upon admission.
Key Responsibilities :
Conduct interviews with patients and family members to gather an accurate list of medications prior to hospital admission.Contact community pharmacies or medical offices for clarification when necessary.Document medication histories and update electronic health records with complete accuracy.Communicate any unresolved issues regarding medication history to pharmacy specialists.Assist in preparing educational materials for patient discharge counseling.Maintain compliance with ongoing education and certification requirements.Participate in regular staff meetings and collaborate with team members.Qualifications :
High school diploma or GED is required.2 years of retail pharmacy experience is mandatory; inpatient pharmacy experience is preferred.Active pharmacy technician or pharmacy intern license is required.Board Certification as a Pharmacy Technician is preferred.We encourage you to apply if you are ready to make a difference as part of our incredible team!
