California Licensed Case Manager RN

A company is looking for a Part Time Case Management, RN. Key Responsibilities Conduct comprehensive assessments of patients' physical, psychosocial, emotional, spiritual, environmental, and financial needs Develop strategic treatment plans in collaboration with patients, caregivers, and multidisciplinary healthcare providers Monitor and evaluate patient outcomes, ensuring effective intervention and quality patient care Required Qualifications Graduation from an accredited Registered Nursing (RN) program Current California RN license; multi-state license also required Minimum 5 years of medical / surgical or acute care experience, including 2 years in case management Knowledge of medical claims and relevant coding (ICD-10, CPT, HCPCS) Ability to work independently and manage multiple cases effectively
