As the intake Coordinator, you will be an integral part of the person-centered team and will be responsible for the coordination and facilitation in the development and implementation of person-centered plans. The Intake Coordinator / Social Services Specialist is the primary contact and liaison for families, referral sources and other stakeholders.

Actively recruit and facilitate the admission of consumers into the Adult Day Program and Group Homes.Coordinate and facilitate in the development and implementation of person-centered plans.Facilitate each consumer’s access to entitlements, benefits, natural supports and available community resources.Ensure compliance with local, state, federal laws and agency regulatory standards and practice.Serve as a liaison with family members, governmental agency representatives and other agency personnel.Act as a point of contact for families / participants with regard to services offered by the agency.What are the requirements?
Bachelor’s Degree in the Social Services field1-3 years related work experience with individuals with developmental disabilities.When can you start? We are looking for new team members to start ASAP! We have a full time shift available RIGHT NOW so come join the Arc Broward family!
