Job Title : Community-Based Physical Therapist – $8,000 Retention Bonus!

Company : Home Health Care, Inc.

Location : Twin Cities Metro area

Compensation : $100,000 - $200,000 annually + $8,000 Retention Bonus!

Tired of the same clinic walls every day? As a Community-Based PT , you'll bring impactful therapy services to patients in their homes—improving mobility, independence, and quality of life.

What You'll Do :

  • Travel to client homes and provide physical therapy services

  • Coach patients and caregivers on exercises and modifications
  • Communicate with nurses, case managers, and primary care providers

    • Requirements :

  • MN Physical Therapy license in good standing
  • Strong interpersonal and adaptability skills
  • Valid driver's license and reliable transportation
