Responsibilities:

• Conduct patient evaluations and develop individualized treatment plans
• Provide physical therapy treatments and interventions
• Document patient progress and communicate with healthcare team
• Educate patients and caregivers on therapy exercises and techniques
• Ensure compliance with all regulatory and company policies

Requirements:

• Valid Kansas or Missouri physical therapy license
• Experience in home care or geriatric physical therapy preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
