Responsibilities

Please note this is a grant funded position that will take place between 9/1/24 - 8/31/25

GENERAL SUMMARY:

• Coordinate case management activities related to the NJSAVE program, including additional support for applicants with high social needs.
• Offer applicants internal support, including scheduling of primary or specialty care appointments, scheduling of behavioral health appointments, introductions to inpatient and ambulatory social work programs, Jefferson Health Plans, and other related services.
• Provide applicants with external referrals to social services, drawing upon Jefferson’s social work database, the Community Outreach team’s database of partners, and new partnerships created through the project.
• Host educational programs for target communities to recruit applicants for NJSAVE.
• Ensure compliance with grant guidelines and internal policies, including social work protocols.

Qualifications

MINIMUM KNOWLEDGE, SKILLS & ABILITIES:

Required Skills:

• Bilingual (English/Spanish) candidates preferred.
• Attention to detail, strong organizational skills, good follow-up.
• Computer literacy; must be able to navigate NJSAVE website, reference and share online resources, and access and submit applications on behalf of applicants.
• Excellent verbal communication skills; must have the ability to collaborate with community leaders to establish partnerships, with colleagues to have applications submitted, and to promote NJSAVE through tabling opportunities and educational workshops.
• Strong understanding of data collection and analysis.
  

Required Education:

• Bachelor's degree in social work.

Conditions of Employment

Covid Vaccination is a requirement for employment at Jefferson for employees working at Jefferson’s clinical entities or at the University. If you are not currently vaccinated you will be required to receive the vaccination prior to hire date if you are offered employment, unless you request and receive an approved medical or religious exemption from Jefferson.
