Job descriptionDeliver home-based care to patients recovering from illnesses, injuries, and chronic diseases, often post-hospital discharge, under RN supervision. Collaborate with a dedicated team of physicians, RNs, CNAs, and aides to ensure quality patient care. Monitor patient vitals, conduct assessments, document progress, and communicate changes to your RN supervisor. Assist RNs in implementing patient care plans and teaching self-care techniques. Educate families on care plans, medication, and home safety. Work with an interdisciplinary team to coordinate patient care and plan for discharge. We are a family-owned and operated organization, fostering a supportive work environment. Our team is made up of collaborative leaders, dedicated clinicians, and supportive office staff, all focused on delivering the best one-on-one care. Your work-life balance is just as important to us as it is to you! Active, unrestricted LPN license in the State of Michigan. At least one (1) year of clinical experience as an LPN. Current CPR Certification. Reliable transportation, valid state driver's license, and current car insurance.
