Role :
The Social Work Manager for the Elder Service Plan (ESP) leads a dynamic, diverse, and skilled group of social workers caring for frail elderly patients in our communities. The Social Work Manager provides supervision to the ESP Social Workers and manages the functions of social work department which includes establishing and updating caseloads and reviewing Social Workers’ documentation. The Manager also maintains their own caseload (20%) which consists of performing assessments, counseling ESP participants, family / caregivers and providing case management on site or in patients home setting. Ideal candidates for this role are experienced in case management with a passion for the elderly and an enthusiasm for collaboration. Position is Monday through Friday with 8 : 30 am-5 : 00 PM with hybrid work options to be determined.
Responsibilities :
Required / Preferred Experience, Education and Skills :
o 2 years of clinical supervisory experience
o 2 years of experience providing therapy in individual, family or group modalities
o 1 year of experience working with elderly (required) and disabled population
