Responsibilities

• Hours of Operation : M-F 4pm-10pm & S / S 10am-6pm
• Provide skilled nursing services to patients under the direction of a physician or midlevel provider, and provides a variety of related services to maintain a safe, therapeutic environment.
• Collect patient health data, analyze the data to determine diagnoses, identify expected outcomes individualized to the patient,
• Develop a plan of care that prescribes interventions to attain the expected outcomes,
• Implement the interventions identified in the plan of care, and evaluate the patient's progress toward attainment of the outcomes,
• Work collaboratively with the healthcare team.
• Perform professional nursing work of considerable difficulty including the care of patients as outlined by physicians and the objectives and policies of the organization.
• Provide patient and family health education and health assessment.
