210 bed facility

Level 2 Trauma Center

This facility is looking for two weeks of coverage per month November 18th ongoing

Inpatient and outpatient need

Schedule : M-F 8-5

Weekend Call : 1 : 3 per month for weekend call

Weeknight call : The provider will be on night call 1-2x per week when working

30 hours per week in the clinic

10 hours per week in the OR

Average of 20-25 patients per day in the clinic

Cases : Bread and butter ENT; ear, larynx, nasal / sinus, throat, peds (ear tubes, tonsillectomy, etc.)

EMR : EPIC

30-45 days to privilege

Candidates must be Board Certified

This facility will consider IMLC candidates if the candidate if they do not have an active Illinois license

$260 per hour with an 8 hour guarantee M-F

$375 per weeknight of call

$2080 (including four hours of gratis) per 24 hour call on Saturday and Sunday

$312 per hour for any hour worked over the 8 hour guarantee M-F, any hour worked in the facility during weeknight call, or any hour over the four hours of gratis working in the facility during 24 hour call on the weekend
