What Youll Do in This Role :
The Director of Clinical Services oversees the delivery of high-quality participant-centered care across all PACE settings. This role ensures clinical operations align with the PACE model promoting independence health and well-being for older adults. The Director collaborates with the interdisciplinary team to integrate care manage clinic operations ensure compliance support quality improvement and develop clinical staff. This leader plays a vital role in maintaining program integrity ensuring participant safety and driving excellence in care delivery and outcomes.
Primary Responsibilities :
Clinical Leadership & Strategy :
Provide strategic direction for clinical operations ensuring compliance with regulatory standards and a strong focus on participant safety.Partner with senior leadership and interdisciplinary teams to design and implement integrated care models that meet the evolving needs of participants.Staff Leadership & Development
Lead recruitment onboarding and professional development for clinical staff while fostering a culture of collaboration accountability and excellence.Oversee performance management and workforce planning to maintain high-quality care and operational efficiency.Quality Compliance & Oversight
Collaborate with the Quality Department to drive quality improvement initiatives and ensure compliance with CMS and other regulatory standards.Monitor clinical outcomes implement corrective actions and manage clinical data reporting to continuously improve care quality.Interdisciplinary Team (IDT) Coordination & Participant Care
Ensure effective collaboration among IDT members for timely assessments care planning and service delivery.Oversee development and implementation of person-centered care plans that promote optimal health outcomes and participant satisfaction.Resource & Budget Management
Manage clinical budgets and allocate resources to support cost-effective operations and high-quality care.Monitor utilization trends and adjust staffing or services to align with organizational goals.Participant & Family Engagement
Serve as a liaison for participants and families addressing concerns and providing education on care plans and available services.Foster a culture of dignity respect and transparency in all participant interactions.Emergency Preparedness & External Collaboration
Develop and maintain emergency preparedness protocols to ensure staff readiness and participant safety during emergencies.Build and maintain relationships with external providers regulatory agencies and community partners to support seamless care transitions and compliance.What Youll Need :
Bachelors Degree in Nursing (BSN) required; Masters Degree in Nursing Healthcare Administration or related field preferred.Current Registered Nurse (RN) license in the state of practice free of disciplinary actions.Certification as a Nurse Practitioner (ANCC or AANP) or graduation from an accredited Physician Assistant program desired.Certification in gerontology case management or healthcare leadership is a plus.CPR Certification or willingness to obtain upon hire.Minimum of 5 years of clinical experience including at least 3 years in a leadership or managerial role.Experience in elder care long-term care home health hospice or a PACE program strongly preferred.Proven experience managing teams and coordinating patient-centered care.The above is a summary of the position it in no way states or implies that these are the only duties this position will be required to perform.
