Milton Nursing & Rehab Center is seeking a dedicated Full-Time PTA to join our compassionate and collaborative rehab team! If you are passionate about helping patients regain strength, mobility, and independence in a supportive SNF / LTC environment, we want to meet you.

Responsibilities

Provide treatment to residents under the direction of a licensed Physical Therapist

Assist in carrying out individualized therapy plans to improve mobility, function, and quality of life

Document services accurately per company, state, and federal guidelines

Collaborate closely with PTs, other rehab staff, nursing, and interdisciplinary team members

Educate residents and families on home exercises and functional strategies

Track patient progress and communicate updates to the therapy team
