Overview
The Associate Healthcare Director (AHCD) is responsible for assisting the communitys Healthcare Director (HCD) in leading the health and wellness activities for the communitys resident population and assuring that the resident care department operates efficiently and effectively. The AHCD will assist in leading the team of medication technicians and nursing assistants in the provision of quality care and services while maintaining compliance with all applicable local, State, and Federal Regulations and Harmony Senior Services policies and standards.
Responsibilities
- Evaluates residents prior to move-in to determine whether their care and service needs can be met in the community. Conducts re-evaluations as indicated by the residents condition and / or State regulations.
- Develops a service plan unique to each resident based on the evaluation and the residents stated preferences. Revises the service plan as indicated by the residents condition and / or State regulations.
- Collaborates with each residents physician, pharmacist, third party healthcare providers, and other members of the healthcare team to promote coordinated care and services.
- Participates in the oversight of the communitys medication management program. Collaborates with the pharmacy, conducts med tech training and skill validation, performs routine audits and other activities that promote safety in medication administration.
- Maintains a working knowledge of all applicable local, State, and Federal regulations.
- Assures that resident evaluations and documentation meet all requirements and collaborates with the HCD to assure that the resident care department is survey ready at all times.
- Participates in the development of, and compliance with, plans of correction as applicable.
- Assures that infection control standards are maintained to prevent or limit the spread of communicable infectious illness.
- Provides training and skill validation for med techs, personal care aides and others to promote safe resident care.
- Participates in the hiring process as needed by conducting interviews and providing onboarding training and coaching to new resident care team members.
- Participates in community and resident care meetings as needed when the HCD is unavailable.
- Provides leadership that promotes a culture of collaboration and teamwork.
- Establishes and maintains a collaborative and positive working relationship with all community team members.
- Communicates compassionately and regularly with residents families. Promptly responds to questions or concerns and participates in effective problem-solving.
- Completes all State and Harmony initial, annual and other required training.
- Partners with the Harmony Square Director and Healthcare Director on managing Behavior Expressions and the overall wellness needs of the Harmony Square residents.
- Carries out other duties as assigned.
Qualifications / Skills / Educational Requirements
Level of formal Education : Current valid and unencumbered license to practice as an RN or LPN in the State in which the community is located.Areas of Study : Nursing. Experience in caring for geriatric individuals and those with Alzheimers and other forms of dementia strongly preferred.Years and type of Experience : 1-2 years experience required; 3 or more years' experience preferred.Special Certificates : Current CPR certification required.Strong organizational and time management skillsEffective verbal and written communication skills.Ability to work a flexible schedule including day, evening and night shift and weekends.Ability to be on-call after hours.Ability to act as the Manager-on-Duty on selected weekends.Why Harmony?
