California RN Care Coordinator

A company is looking for an RN Care Coordinator to provide clinical care coordination for patients.

Key Responsibilities

Provide clinical oversight and ensure compliance with documentation standards for care plans

Serve as the primary clinical contact for patients, coordinating care across various services

Participate in quality improvement initiatives and monitor compliance with clinical benchmarks

Required Qualifications

Active, unrestricted RN license in California; multi-state license is a plus

Minimum of 3 years of clinical experience in community-based care or care coordination

Familiarity with Medi-Cal, ECM, and population-based care models

Experience working with interdisciplinary teams, including CHWs and behavioral health clinicians

Knowledge of health plan portals, reporting systems, and EHRs
