Title : Home Care CNA Supervisor
Location : Lafayette, CO
Schedule : Monday-Friday, 8am to 5pm
Pay : $62,788 - $77,176 per year
JOB SUMMARY
Under supervision of the Home Care Services Manager, the CNA Supervisor directs CNA care and services in the community for the PACE participants. Responsibilities include planning of caregiver services, coordination of cares, and scheduling the CNA team for their visits. Works in conjunction with the home care nurses to implement appropriate caregiver services identified as necessary for supporting TRU PACE participants so they can remain living independently in the community for as long as possible.
EDUCATION AND QUALIFICATIONS
- College degree in a field such as rehabilitative or recreational therapy, nursing, social work, or related field, highly preferred. May substitute Certified Nurse's Aide with 2 years relevant experience.
- 2+ years of additional work experience in healthcare
- 1+ years of experience with scheduling responsibilities
- 1+ years of supervisory or management experience
- Knowledge of health care delivery systems, regulations, quality assurance and medical and non-medical needs of the elderly
- Must possess CPR certification or be able to obtain within 60 days of hire
- Must successfully pass agency required background checks and drug screening
ESSENTIAL DUTIES
Directs, supervises, and evaluates the performance of the Home Care CNAs.Assures that Home Care CNA initial and annual competencies are scheduled and completed within timeframes.Provides / coordinates in-services and training for Home Care CNAs according to program needs and state and federal regulatory requirements.Ensures that the job responsibilities, authorities and accountability of all direct reports are defined and understood.Order incontinence and other associated supplies for participants for home deliveries.In conjunction with the Home Care Services Manager, responsible for maintaining certification under CMS state and federal program requirements, obtaining and maintaining certification under CMS as well as any other state or federal law or rules.Participate as a member of the interdisciplinary team, maintaining regular attendance at and participate in interdisciplinary team meetings; communicate participant changes, collaborate on care planning decisions and coordination of 24-hour care delivery.Actively communicate relevant issues to the team, ensures all team members are informed and up to date, participates in the development of the plan of care and monitors and articulates participant's progress and decline.TRU's Benefits (Eligibility Varies by Position / Status) :
