Adult Intensive Services (AIS) serves clients experiencing Severe and Persistent Mental Illnesses (SPMI) who need a higher level of care than traditional outpatient services may provide. AIS provides responsive, “wrap around” support including individual therapy, case management, psychoeducation, group therapy, and crisis intervention services in virtual, office, and community settings. In addition, AIS provides coordination of care, consultation, and advocacy with internal programs, external agencies, community resources, family members, and other client supports. This is a dynamic and exciting position which requires strong clinical, communication, and problem-solving skills along with a creative, responsive, and trauma informed approach to care. AIS offers many opportunities for growth and learning as well as a flexible schedule that combines time working from home with time working in the office and in the community. This position is based out of Idaho Springs & Evergreen and serves clients living in rural Jefferson, Clear Creek and Gilpin counties.
Education, Knowledge, Skills & Experience :
- Master's degree in social work, counseling, psychology, or related field required
- Registration with DORA required (LSW, SWC, or LPCC if not yet licensed)
- LPC, LP, or LCSW preferred but not required
- CAS or LAC helpful but not required
- Licensure supervision provided for LCSW, LPC, and LAC candidates
- Driver’s License Required. Motor Vehicle Insurance Coverage requires applicant must be 25 years or older. Must have an acceptable Motor Vehicle Driving record as determined by our insurance carrier. An MVR will be pulled along with a background check.
Job Specific Competencies / Duties :
Provide therapy and case management services including service planning, monitoring, follow-up, linkage, and crisis management for consumersProvide services in an office, the community, virtually, or in the consumer’s homeMake appropriate referrals to and act as liaison with community agencies, other service providers, and natural support systemsCollaborate with consumers, family members, and other supports in identifying strengths, cultural factors, and recovery needs for developing personalized treatment goalsKnowledge of Stages of Change model, Motivational Interviewing techniques, risk assessments, Person Centered treatment approaches, Trauma Informed care, and Cognitive Behavioral Therapy skills, and Recovery-Based Treatment practices is essential. Knowledge of DBT skills helpful but not requiredProvide assistance in developing or connecting to resources for developing social, vocational, spiritual, cultural, and leisure abilities as well as basic needs such as housing, food, clothing, medical and dental care, legal, vocational, and psychiatric servicesMaintain communication and care coordination with other Jefferson Center programsEngage in ongoing monitoring of consumers on assigned caseload including assessment of symptoms to prevent relapse, assessment of daily living skills (ADL’s), engagement outreach, hospital discharge planning, substance use assessment, and respite service needsProvide services and related duties for clients on 27-65 Certification to treatmentAssess clients for risk / lethality and coordination with crisis or emergency services when clinically indicated / medically necessary, including hospital admissions and M-1 holdsAssist with applications for benefits in collaboration with Benefits / Navigation TeamParticipate in team meetings, clinical staffings, and clinical consultations as scheduledFollow ethical and clinical practice guidelines as well as Center and State Division of Mental Health standards to provide appropriate service delivery and maintain standards of careParticipate in individual and group supervision. If unlicensed, track hours pursuant to attaining licensure.Assist with special projects or Center committees as indicated, desired, or assignedOther Duties (Professional Growth / Development / Community / Relationships & Productivity Performance Measures) :
Attend mandatory in-services, compliance with individualized training plan as requiredUse language and behavior that promotes dignity / respect with consumers, Center staff and external customersProvide peer consultation and leadership to support clinical / professional growth of other team members, unlicensed staff, and the team as a whole and assist with training / mentoring new hires as appropriate if licensed.Satisfactorily complete 90% of peer review charts needing corrections within 14 calendar daysComplete a minimum of 90% of all paperwork by the due dateComplete a minimum of 90% of case closing within specified time framesMeet required number of consumer service hours as determined by Network Director and / or Clinical ManagerExhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environmentMaintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors, community partners, and the general publicDemonstrate cultural competency / awareness / skills in working with diverse caseload and utilize Center resources / trainings as neededEffectively respond to client / consumer needs and problems, initiate and maintain positive interactions, and provide timely response to phone calls, email, messages, etc.Note : Employees are held accountable for all duties of this job. Salary Grade 51 - Master’s Unlicensed Clinician $57,000 - $64,000
Salary Grade 52 - (Licensed Clinician LCSW, LMFT, LPC) $70,000 – $77,200
Additional Salary Information
The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours / week will be prorated and adjusted to the appropriate FTE.Application deadline : 10 / 30 / 2025. Review of applications will begin immediately.
