Description
Central Valley Indian Health, Inc. (CVIH) is seeking a compassionate, reliable Community Health Representative. This person will serve patients in areas including : Dunlap, Tollhouse, Auberry, Yokuts Valley, and more. This position provides paraprofessional community-oriented primary health, disease prevention services and traditional native concepts in multiple settings within the CVIH service area. Candidates must be comfortable driving in rural areas and visiting patients' homes. This is a "Safety Sensitive" position, responsible for HIPAA compliance.
PRIMARY RESPONSIBILITIES :
A. Provide curative, preventive and rehabilitative services to all Indian people living in the CVIH service area.
B. Relate to and have knowledge of people in the community. Facilitate communication between community members and health care providers.
C. Build and maintain the trust & confidence of clients and health care providers.
D. Act as client advocate to facilitate services to clients and assist in overcoming barriers to acquisition of health care.
E. As a case finder, refer patients to Public Health Nurse for services.
F. Prepare and maintain a visitation schedule with all eligible residents of the service area.
G. Maintain a follow-up schedule, including clinic appointments.
H. Prepare and maintain family profiles on service population. Conduct ongoing review and assessment of each client / family profile.
I. Transport patients to CVIH and other health care agencies in accordance with CVIH protocols and individual work plans.
J. Promptly report and document any patient or vehicle issues.
K. Work with other Outreach staff to facilitate timely and positive patient outcomes.
L. Attend workshops and training sessions as offered.
M. Transport supplies, records, interoffice mail, etc. between clinics as needed.
N. Communicate in a friendly and professional manner to others. Provide excellent service / support to all patients, employees, and outside contacts.
O. Any other duties assigned to the employee by his or her direct supervisor or CVIH Administration.
Requirements
1. 1. High school diploma or GED.
2. Live within and / or have knowledge of service area.
3. Must be able to carry & lift supplies and / or equipment weighing 50 lbs.
4. Must be able to sit for extended periods of time.
5. A valid California Driver's License and clean driving record with the ability to be insured under CVIH's automobile insurance.
6. Must be able to read, write, follow instructions, prepare reports and demonstrate effective communication skills.
7. Current CPR and First Aid training.
8. Requires an awareness and keen appreciation of Indian traditions, customs, and socioeconomic needs; an ability at times to meet and deal effectively with Indian health organizations consisting of numerous tribes with varied cultures which requires tact, courtesy, discretion, resourcefulness, and good judgement in handling functions of a sensitive nature.
9. "Safety Sensitive" positions are subject to random drug screening and is required as a condition for continued employment.
