Job description

NEW : LCSW Needed : Just 1 hour south of the Portland area

Job opportunity includes :

  • Working with individuals to provide a full assessment from a biopsychosocial perspective to address each person's overall health.
  • Providing services in a location that is most comfortable for the individual
  • Developing a person-centered plan with each participant to promote engagement in self-management activities in coordination with case manager.

Details include :

  • Full Time Exempt 40 Hours

    • Shift : Monday - Friday

    Salary Scale $5794.53 to $6267.73 Monthly

  • Must be licensed as Licensed Clinical Social Worker (LCSW) or
  • Licensed Practicing Counselor (LPC).
