LPN or RN to take care of toddler

Job Summary

We are seeking a compassionate and dedicated LPN or RN to provide one-on-one in-home nursing care for an active, cheerful toddler in Rockford, IL (61107). The child is trach- and G-tube dependent, receiving all nutrition and medications through the G-tube. This role involves delivering skilled, attentive care while supporting a fun, engaging daily routine-whether watching Ms. Rachel and Lucas and Friends or enjoying playtime on the jungle gym.

Schedule : Monday-Friday, 7 : 00 a.m.-5 : 00 p.m.

Position Type : Full-time, in-home private duty nursing

Responsibilities :

• Provide skilled nursing care according to the care plan
• Monitor health status, track vital signs, and administer medications / treatments
• Collaborate with family and care team to ensure the best outcomes
• Maintain accurate charting and documentation
