Department Details

The Skilled Rehab and Home Health team at Good Samaritan is excited to hire a dedicated Physical Therapist. This full-time position involves 35-40 hours per week, working Monday through Friday, with flexibility for weekends and holidays. You will cover the Skilled Nursing Facilities and surrounding Home Health clients. Some of the conditions we treat include orthopedic issues, weakness, balance / falls / gait problems, cervical / back pain, general deconditioning, and more. Our current team consists of 1 full-time PT and 2 full-time PTAs. New graduates are encouraged to apply as we seek a passionate individual to complement our team. Travel expenses and mileage will be compensated for visits to home health patients.

Job Summary

As a Physical Therapist, you will :

• Plan, organize, and deliver physical therapy programs to help clients improve mobility, relieve pain, increase strength, and prevent deformities.
• Conduct examinations, evaluations, and interventions for clients impacted by injury or disease.
• Utilize various tools and techniques to implement safe physical therapy interventions.
• Document client information meticulously and establish therapeutic plans, modifying them as needed.
• Educate clients and their family members about treatment plans and self-exercises for home care.
• Demonstrate a comprehensive understanding of current research and apply it in daily treatment.
• Work with individuals of all ages with diverse diagnoses and disorders, maintaining documentation according to departmental guidelines.
• Exhibit flexibility regarding variable work schedules and collaborate with various stakeholders, including providers, case managers, and third-party payers.
• Supervise assistants, aides, and students, and provide ongoing education and training to peers.
• Perform any other related duties as requested by the manager.

Depending on facility needs, you may focus on interventions for clients in post-acute, long-term care, and home health care settings.
