UPMC Family Hospice Home Care Aide

UPMC Family Hospice is hiring a Full-Time Home Care Aide for the State College and surrounding areas. Hours are Monday-Friday 8 : 00 am-4 : 30 pm.

Responsibilities include :

• Communicating appropriately with the interdisciplinary home care team promptly, reporting any changes in patients' condition to appropriate professional staff.
• Attending compliance training and adhering to the organization's standards of conduct, policies, and procedures.
• Focusing on customer service and continually striving to perform the duties of their job in a manner that will result in optimal patient satisfaction.
• Providing safe and direct personal care to patients in the home setting under the direct supervision of an appropriate professional.
• Performing duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
• Identifying, developing, and participating in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
• Proven ability to facilitate change.
• Maintaining and completing documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
• Participating in all required agency meetings.
• Maintaining annual competency evaluation scores as determined by regulatory requirements.
