• Assess patient needs and implement individualized care plans.
• Coordinate discharge planning and patient education.

Requirements :

• Licensure, Registration, and / or Certification : Valid Registered Nurse license and BLS certification from AHA.
• Education : Graduation from an accredited nursing program.
• Experience : 1 year of clinical nursing experience preferred, but new grads are welcome to apply.
