Relocation Assistance & Bonus : Available

Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM (No on-call or after-hours)

Responsibilities

• Deliver clinical care in Nursing Homes, Assisted Living, and Memory Care facilities
• Collaborate with a multidisciplinary team to ensure quality outcomes
• Perform evaluations, manage treatment plans, and monitor patient progress
• Maintain accurate documentation and communicate effectively with care teams
