About The Position
Job Title : Care Manager

License : Current full, unrestricted Registered Nursing license in Alaska
Certifications : Basic Life Support (BLS) certification preferred
Must-Have : Active Alaska nursing license, ambulatory care experience, proof of flu vaccine, fingerprint cards
Description :
This position functions within the urgent care setting, providing appropriate and timely nursing care for patients of all ages.
The focus is on promoting quality, evidence-based, and cost-effective health care outcomes.
This position works within a nursing team consisting of RNs, LPNs, and Clinical Assistants.
The RN will perform full assessments, prioritize patient needs and patient flow, perform skills, and coordinate care as ordered by providers.
The RN is expected to use critical thinking to work within standing orders to expedite care and patient services.
As necessary, the RN may work in other nursing roles to ensure daily departmental functions are covered.
Perform duties in a responsible, professional, and compassionate manner to provide excellent patient care.
Key essential functions include performing initial triage and assessments of patients presenting for urgent care services.
The RN also provides telephone services, including assessments and advice on self-care and appropriate care options.
Assessment includes history of present illness, vital sign measurements, and gathering of necessary health data.
The RN may take direction from providers through written, verbal, and standing orders.
Administer therapeutic measures, including medication, and oversee specialized equipment as indicated by patient needs.
Initiate IV access, maintain the site, monitor patients, and discontinue IV therapy as necessary.
Document assessments, observations, nursing interventions, and health education provided to patients in person or by phone.
Evaluate and document patients' comprehension of education provided.
Maintain a clean and safe working environment, ensuring adequate supplies and equipment.
Report any defective or missing equipment and safety hazards immediately.
Adhere to infection control policy and procedures.
Perform additional duties as assigned, including special projects necessary to provide comprehensive health services.
Facilitate coordination of patient referrals, track referral progress and outcomes, and provide periodic status reports to providers.
Serve as the primary link for case managers and referral coordinators in care coordination for patients.
Proactively identify delays and obstacles to care and work to remedy individual cases and improve the overall process.
Contact patients for follow-up calls and handle each situation appropriately.
Build bridges and create rapport with team members within departments and across the organization.
Participate in team improvement efforts and continued development of the clinical team.
Participate in triage role and respond to emergency situations as necessary.
May serve as a vaccine coordinator or secondary vaccine coordinator for the clinic.
Perform additional duties as needed.
Education and experience requirements include a graduate of an accredited nursing school.
Knowledge of customer service principles, safety and infection control, and clinic protocols is crucial.
Skills in assessment, data collection, communication, and life-saving procedures are essential.
The ability to work independently while being part of a team and manage multiple tasks is important.
Physical demands include the ability to talk or hear regularly, stand, walk, and lift / move 50 lbs.
The position requires remaining calm, professional, and focused during stressful situations.
