Position : Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC) Locations : Across our five sites, including clinics and local schools in New Hampshire Schedule : Flexible—full-time or part-time available. Core hours are 8 am–5 pm, with flexibility around start / end times to accommodate patients. Work Breakdown : Approximately 80% clinical (direct care), 20% administrative tasks Key Responsibilities : Develop, monitor, and coordinate behavior management plans with the clinical team. Provide direct therapeutic care to patients—especially children—in clinic and school‑based settings. Uphold excellent service quality and facilitate clear care coordination with patients, clinical staff, and referral sources. Contribute to performance improvement projects. Investigate patient backgrounds and collaborate with physicians and care teams on diagnostics and treatment. Educate and support families to ease barriers to psychiatric care and encourage treatment adherence. Connect families to community resources and reinforce compliance with medical recommendations. Support and train staff on mental health and substance abuse issues. Document care plans and patient progress in the Electronic Medical Record (EMR) using objective, measurable methods. Participate in clinical team meetings and draft psychosocial reports for patients’ Individual Plans of Service. Deliver age‑appropriate care, leveraging an understanding of lifespan developmental stages. Accept other duties, hours, or site assignments as needed. Remain current in best practices and therapeutic approaches.
