Clinical Care Coordinator

A company is looking for a Clinical Care Coordinator.

Key Responsibilities

Connect clients and families to appropriate clinical resources and oversee care coordination efforts

Provide advanced case management and social work support, including navigating FMLA and school accommodations

Lead the outbound referral process and maintain documentation in client medical records

Required Qualifications

Bachelor's degree or higher education

2 years of experience with eating disorder clients and families preferred

Prior case management experience preferred

Computer literacy and ability to manage referrals through Salesforce

Understanding of diverse cultures and gender-specific issues
