Job descriptionPatient Assessment : Evaluate patients’ physical conditions, mobility, and functional abilities to develop comprehensive treatment plans tailored to their needs. Treatment Planning : Create and implement personalized therapy programs that address specific goals, improve mobility, alleviate pain, and enhance overall physical function. Therapeutic Interventions : Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals. Progress Monitoring : Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes. Patient Education : Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance. Collaboration : Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress. Documentation : Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements. Compliance : Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment. Education : Doctoral or Master’s degree in Physical Therapy from an accredited institution. Licensure : Current state licensure as a Physical Therapist. Experience : Previous experience in a skilled nursing or long-term care setting preferred, but not required. Personal Attributes : Compassionate, patient-focused, and dedicated to providing high-quality care. Strong organizational skills and attention to detail are essential. Strong clinical assessment and treatment skills Effective communication Ability to work as part of a collaborative healthcare team.
