Role Description

This is a full-time hybrid role for a Licensed Practical Nurse located in Chattanooga, TN, with some work-from-home flexibility. The Licensed Practical Nurse will be responsible for providing home-based care, conducting patient assessments, administering medications, monitoring patient health, and collaborating with other healthcare professionals. Additional tasks include patient education on managing chronic illnesses, updating care plans, and ensuring compliance with healthcare regulations.

Qualifications

• Licensed Practical Nurse (LPN) certification and current licensure in Tennessee
• Experience in providing home-based healthcare and conducting patient assessments
• Proficiency in administering medications and monitoring patient health
• Strong communication and patient education skills
• Ability to work independently and within a team
• Experience with electronic health records (EHR) systems
• Knowledge of chronic disease management and healthcare regulations
• BLS (Basic Life Support) certification
• Reliable transportation and willingness to travel for home visits
• Excellent organizational and time management skills
