Description :

This position involves conducting initial in-home evaluations of patients.

The evaluations will assess health status, strengths, care needs, and preferences.

Nurses will guide the development of individualized long-term care service plans.

Candidates must review consumer medical documentation or health referral forms as relevant to the case.

Evaluation data must be entered into an electronic evaluation form and transmitted as required.

Any concerns, conflicting information, and other issues that arise during the evaluation process should be documented.

The role emphasizes continuity of care to reduce fragmentation and gaps in treatment plans.

Candidates will discuss healthcare options, needed supports, service vendor options, and waiver options with consumers.

Communication should be initiated between the call center and eligible consumers for enrollment into managed care plans.

Enhancing communication and collaborative relationships with interdisciplinary care team members is essential to improve care coordination.

Collecting quality review data and required documentation to support outcome measurements is part of the role.

Maintaining knowledge of community resources and payor requirements for the target population is necessary.

The position requires meeting all established standards as outlined in the annual performance criteria.

Other duties may be assigned by the Regional Nurse Manager or project management.
