Oregon Licensed Clinical Program Coordinator RN

A company is looking for a Clinical Program Coordinator RN, Medicare / Medicaid who will provide care coordination and case management services to members.

Key Responsibilities

Provide care coordination, case management, and care management services to members with acute and complex health care needs

Implement disease management programs, educating and empowering members to manage their conditions

Assist with managing member health plan benefits and improving access to quality care

Required Qualifications

5+ years of clinical nursing experience

Experience collaborating with physicians in patient care management

Current unencumbered Registered Nurse License in state of residency
