Monday–Friday | 8 : 00 AM–4 : 30 PM | No Weekends or On-Call
Registered Nurse (Hospice / Home Health) About the Role
We’re seeking a compassionate and experienced Registered Nurse (RN) to join a respected Hospice and Home Health team serving Lancaster and York Counties . This is a rewarding opportunity to provide quality, end-of-life care and ensure patients and families receive the highest level of support and comfort.
You’ll work independently in the field, performing comprehensive assessments and visits while collaborating closely with physicians, social workers, and care teams to deliver patient-centered outcomes.
Registered Nurse (Hospice / Home Health) Position Highlights
- Schedule : Monday–Friday, 8 : 00 AM–4 : 30 PM
- No weekends or on-call
- Coverage Area : Lancaster and York Counties
- Mileage Reimbursement : Provided
- Productivity Benchmark : 5–7 visits per day
Registered Nurse (Hospice / Home Health) Primary Responsibilities
Perform comprehensive patient assessments and care planningComplete admission, recertification, routine, PRN, and death visitsCollaborate with physicians, discharge planners, and interdisciplinary team membersCoordinate transitions of care and ensure appropriate levels of support and interventionMaintain accurate documentation in HomeCare HomeBase (HCHB)Provide compassionate end-of-life care and patient / family educationPromote health awareness and symptom management to prevent complicationsServe as a patient advocate while maintaining confidentiality and compliance standardsRegistered Nurse (Hospice / Home Health) Qualifications
Active RN License (State of Pennsylvania or Compact License accepted)Hospice experience requiredHomeCare HomeBase (HCHB) experience requiredValid driver’s license, vehicle insurance, and reliable transportationCurrent BLS / CPR certificationAssociate’s or Bachelor’s Degree in Nursing or related fieldStrong ability to assess complex medical conditions and make informed clinical decisionsExcellent time management, organizational, and documentation skillsRegistered Nurse (Hospice / Home Health) Preferred Background
