Lancaster & York Counties, Pennsylvania

Monday–Friday | 8 : 00 AM–4 : 30 PM | No Weekends or On-Call

Registered Nurse (Hospice / Home Health) About the Role

We’re seeking a compassionate and experienced Registered Nurse (RN) to join a respected Hospice and Home Health team serving Lancaster and York Counties . This is a rewarding opportunity to provide quality, end-of-life care and ensure patients and families receive the highest level of support and comfort.

You’ll work independently in the field, performing comprehensive assessments and visits while collaborating closely with physicians, social workers, and care teams to deliver patient-centered outcomes.

Registered Nurse (Hospice / Home Health) Position Highlights

Schedule : Monday–Friday, 8 : 00 AM–4 : 30 PM
No weekends or on-call
Coverage Area : Lancaster and York Counties
Mileage Reimbursement : Provided
Productivity Benchmark : 5–7 visits per day

Registered Nurse (Hospice / Home Health) Primary Responsibilities

Perform comprehensive patient assessments and care planning

Complete admission, recertification, routine, PRN, and death visits

Collaborate with physicians, discharge planners, and interdisciplinary team members

Coordinate transitions of care and ensure appropriate levels of support and intervention

Maintain accurate documentation in HomeCare HomeBase (HCHB)

Provide compassionate end-of-life care and patient / family education

Promote health awareness and symptom management to prevent complications

Serve as a patient advocate while maintaining confidentiality and compliance standards

Registered Nurse (Hospice / Home Health) Qualifications

Active RN License (State of Pennsylvania or Compact License accepted)

Hospice experience required

HomeCare HomeBase (HCHB) experience required

Valid driver’s license, vehicle insurance, and reliable transportation

Current BLS / CPR certification

Associate’s or Bachelor’s Degree in Nursing or related field

Strong ability to assess complex medical conditions and make informed clinical decisions

Excellent time management, organizational, and documentation skills

Registered Nurse (Hospice / Home Health) Preferred Background

Experience in Home Health or Hospice Case Management

Compassionate, patient-focused approach to end-of-life care

Skilled in communication, care coordination, and family education
