Key Position Details :

Position supports patients with Breast cancer across the care continuum.

Hybrid position (min. 3 days on site per week)

Full time position (80 hours every two-week pay period)

8-hour day shifts

No weekends

Job Description :

Principle Responsibilities

Provides care management and overall plan of care support to a panel of patients who require integrated oncology services.

Collaborates with the oncology care providers primary care providers and the interdisciplinary care team across the continuum of care.

Engages in quality improvement initiatives and program development.

Required Qualifications

Licenses / Certifications

Current BLS certification from the American Heart Association

Current BLS certification from the American Red Cross

Allina in-house BLS training (within 30 days of hire)

Pay Range

Pay Range : $45.40 to $62.82 per hour

The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including but not limited to your experience education and the union agreement (if applicable). Shift weekend and / or other differentials may be available to increase your pay rate for certain shifts or work.
