Social Services Director

Valley Manor Rehab & Care Center is a 181-bed skilled nursing and rehabilitation facility committed to providing high-quality care and compassionate support to our residents and their families. We are currently seeking a dedicated and experienced Social Services Director to join our leadership team.

Position Summary :

The Social Services Director is responsible for overseeing the planning, development, and implementation of social services programs to meet the emotional, social, and psychological needs of residents. This role works collaboratively with the interdisciplinary team to ensure resident-centered care and a smooth transition through all stages of admission, care, and discharge.

Key Responsibilities :

Plan, organize, and direct the overall operation of the Social Services Department.

Advocate for residents’ rights and ensure compliance with state, federal, and facility policies.

Conduct psychosocial assessments and care planning for residents.

Provide support to residents and families regarding adjustment, emotional needs, and community resources.

Coordinate discharge planning, including referrals to community agencies and services.

Supervise and support social services staff to maintain a high standard of care.

Participate in interdisciplinary care conferences and quality improvement initiatives.

Qualifications :

Bachelor’s degree in Social Work or related field (Master’s preferred).

Licensed Social Worker (LSW) or equivalent state certification required.

Previous experience in long-term care or skilled nursing facility strongly preferred.

Strong leadership, communication, and organizational skills.
