This position provides skilled nursing care to patients in their homes throughout Lake Forest and surrounding areas. The role focuses on delivering compassionate, patient-centered care while maintaining flexibility and autonomy. The nurse will manage visits, documentation, and care coordination within a supportive, team-based environment.

Key responsibilities

Deliver professional nursing care to patients in home settings

Assess, plan, and implement individualized care plans

Complete documentation and charting accurately and on time

Collaborate with care teams to ensure quality outcomes

Manage time and patient schedules independently

Schedule

Monday through Friday, flexible scheduling

Minimal weekend coverage required

Compensation

Pay-per-visit structure

$10,000 bonus paid quarterly

Travel

Local travel required; mileage reimbursement provided

Qualifications

Required

Associate degree or higher in Nursing

At least 1 year of experience as an RN (Home Health experience preferred)

Active Registered Nurse (RN) license in Illinois

Current BLS and CPR certifications (support available if not current)

Valid driver's license, reliable transportation, and current auto insurance

Preferred

RN license number provided for credential verification
