A company is looking for a Care Coordinator for the Eastern Time Zone.

Key Responsibilities

Engage and enroll patients through telephonic outreach, serving as their primary point of contact

Develop and implement care plans in collaboration with the interdisciplinary care team, ensuring seamless transitions of care

Advocate for patients by identifying barriers to care and connecting them with necessary resources and support

Required Qualifications

3 years of experience in healthcare coordination or a related field

Experience with a team-based approach to care

Strong knowledge of community resources

Ability to work effectively in a remote environment with required internet speeds

Intermediate skills with MS Office Suite, including Outlook and Teams
