About The Position

Job Title : RN Case Manager

Experience : Minimum of 2 years in the role of Inpatient RN Case Manager

License : Indiana RN License or compact

Certifications : Preferred certification in Case Management

Must-Have :

• Ability to work independently
• Ability to identify work that needs to be done and follow through to completion without prompting
• Ability to work collaboratively with the care team
• Ability to complete a comprehensive patient assessment including patient status to drive discharge
• Ability to drive stay progression and complete discharge planning to expedite patients' transition to the next level of care
• Ability to document comprehensively, accurately, and timely

Description :

The position involves general job duties that include collaborative work with the Care Team.

Completion of a comprehensive patient assessment is required to drive discharge.

Driving stay progression is essential, along with completing discharge planning in a manner that expedites patients' transition to the next level of care.

Accurate documentation in an electronic health record system is required.

The use of cell phones is expected for communication purposes.

The employee will primarily stay within one unit but may float to another unit as needed to perform Inpatient Case Management work.

Units typically range from 20 to 36 beds, and there may be one or two RN Case Managers assigned to a unit.

Participation in weekend rotation is mandatory.

Business casual attire is required, with close-toed shoes and an option for scrubs.

Training includes a general orientation, specific role training, and time with peers for oversight.
